Whanganui Regional Health Network (WRHN) are seeking a Population Health Programme Coordinator to join the team. This role has been developed from a desire to reduce the inequity in population health outcomes and will work closely with the Practice Facilitators and Business Unit.
This role requires the successful applicant to act as a conduit between contracted general practices and the WRHN to build capacity and capability by leading focused population health activities. You will work in collaboration with general practices to develop trusted and transparent approaches, by supporting data collection processes and analysis to improve practice and population healthcare, which will allow ongoing evaluation to support changes in service delivery.
Working in an exciting fast-paced environment, this role will require you to work alongside WRHN staff, members of the general practices, local community providers and agencies, and local Māori organisations.
The ideal candidate will have:
- A high level of technical skill in health data management and analysis.
- An understanding of population health and equity outcomes in health.
- Relevant primary health experience.
- Specific interest and/or understanding of primary healthcare.
- Excellent written and oral communication skills.
- A good understanding of how general practice operates.
This is a permanent full-time position based within a supportive team environment.
WRHN is located next to Whanganui Hospital and is in very close proximity to the magnificent Whanganui River. Centrally situated, the city is close to many outdoor sporting and recreational locations; including being on the doorstep of the National Park and Central Plateau.
